Fires resulting from leaking fuel sprayed or released onto hot surfaces of diesel engines.
In August 1994 such a fire on a foreign flag ro-ro ferry serving United Kingdom (UK) ports, was investigated by the Marine Accident Investigation Branch (MAIB). The leakage of fuel resulted from the fatigue failure of a Low Pressure (LP) system flange bolt. This led to investigations which revealed that pressure pulses far in excess of the system working pressure were a feature of design. The MAIB recommended in their report of this incident that research should be undertaken to evaluate whether there was a need to introduce more stringent requirements for the fuel pipes of LP systems, to ensure that they could withstand he peak pressures expected, and experienced, in service.
